Asking For Travelling Insurance |

Dear Sir,
I am leaving for Colombo for an on-the-spot study of situation now prevailing on 26th June, 2xxx, and would like to have a travelling insurance policy to the tune of USD500,000.
Please send me the necessary forms for completion and let me know the total amount of premium to be paid. I shall be in Colombo for ten days from 27th June to 7th July, 2xxx.
